Your original password was set up as your last name as maintained in the NCBDE database, and your last name would need to be entered to match the NCBDE database with upper and lower case letters, e.g., "Smith", rather than "smith" or "SMITH", OR "Jones-Jackson", rather than "Jones" or "jones-jackson".
