The 2009 Certification Handbook (and Application) is available via download* (click here for access to downloadable file) and mail. To obtain the 2009 Handbook via mail, send a request including your mailing address, by mail, telephone, facsimile or e-mail to NCBDE's testing agency, Applied Measurement Professionals (AMP). There is no charge for the handbook and application.

If utilizing the Handbook and Application obtained from the web site, use a blank envelope and mail the application, appropriate documentation, and all required fees to:

†IMPORTANT: Be sure to review the Handbook for the dates that applications are accepted. Applications submitted outside of the application window cannot be processed and will be returned.
The application window for the spring 2009 Examination has closed. We hope you will choose to apply for the fall 2009 Examination. The application window for the fall 2009 Examination will be open from July 15 to September 15. Please refer to the Handbook for the details.
There are two ways** to apply for the Examination‡:
**For initial certification and individuals whose CDE credentials lapsed prior to 12/31/2008 ONLY: Individuals applying with an advanced degree (see Eligibility Requirements, 1.C. or 1.D., page 5) must apply using the paper method and submit required official transcripts.In addition, individuals who do not wish to use their social security number as an identifier must apply using the paper application and include a note to this effect. A unique identifying number will be assigned to you when your application is processed.
‡IMPORTANT: Be sure to review the Handbook or web site for pertinent application date windows. Applications submitted outside of the application windows cannot be processed and, if submitted on paper, the applications will be returned.
Ways to Apply
1. Online Application and Scheduling: Complete the application and scheduling process in one online session by visiting www.goAMP.com. Click on “Candidates,” and follow the online instructions. [Note: Certified Diabetes Educators or those individuals whose credentials have lapsed must have their CDE/certificate numbers available
at the time of registration.]Click here to access the online application process. To begin, use the "Register for this Exam" menu option on the far-right hand side of the screen.
2. Paper Application and Scheduling: Complete and submit to AMP a paper application.
