Verification of Certification
Verification information for current CDEs or for individuals who have allowed their credentials to lapse may be obtained in a document available through the NCBDE national office. The verification of certification document will include the certification number, expiration date, and current status of the certification.
Requests for verification of certification from certificants or individuals whose certifications have lapsed
To obtain verification of certification document from the NCBDE national office, submit a signed written request that includes your name, certification number (or last four digits of social security number), current home address, and daytime telephone number with area code.
Note: The verification will be mailed to the certificant's home address unless a facsimile number is provided with the request.
Requests for verification of certification from other interested parties
For an interested party to obtain verification of certification document from the NCBDE national office, submit the following:
- A written request on letterhead indicating the name and last four digits of the social security number and/or certificate number for the Certified Diabetes Educator(s) requiring verification. The request must also include a contact name and daytime telephone number (with area code) for the organization and the mailing address where the verification is to be sent.
- A signed "Consent to Release" form that is not more than one year old.
Note: The verification will be mailed to the organization unless a facsimile number is provided with the request.
